2. Basically, everyone said all they did was fill out this form in various ways. It seems that there really isn't very much write up to do, with most just doing a version of “cut and paste” into this from from their patient's Careweb data.

3. I cold called pathology at Uni (where my case was done) and told the person that picked up I was a radiology resident with a need to take slides from a case to Washington…they directed me to the right person (Becky someone, I'm not sure her name or title) and they seemed glad to help. To get slides together, I just had to fill out half of a one page request form that they faxed to me, and in a week or so they will have slides available for me to pick up.

4. One piece of advice I will offer free of charge: get on the housing chase sooner than later…it can get seriously stressful and costly to wait.

5. Label ALL of your material that you submit with the code provided by the Pathology Dept. They (AFIP) use that to catalog your case information.

6. Be sure to write DICOM on your CD in large letters (assuming you are not submitting a case in hard copy).
